Background:

Julian Luthor is first mentioned in "Stray", season 1. Lex mentions to Clark having had an infant brother who was born when Lex was 11 or 12 and who was found dead in his crib on the morning of his baptism. Lex says, "The day Julian was born was the only time I felt like I was part of a real family". After Julian's death, his mother was never the same, Lionel grew even more distant, having, as Lex sees it, lost his chance to have a son he could love.

In "Shattered", season 3, Clark finds Lex singing to a rolled-up blanket in the barn. Lex says, "It's my little brother, Julian", though we never learn for sure whether this means Lex has taken leave of his senses or whether he is counterfeiting madness to protect himself. Chloe does reveal to Clark that Lex had similar episodes in boarding school after Julian's death.

It isn't until the season 3 episode, "Memoria", that we learn the real circumstances of Julian's death. When Lex undergoes memory-regression treatment at Summerholt Institute, he recalls that his mother, Lillian Luthor, smothered the baby with a pillow and that he, Lex, let his father think that he was the culprit in order to protect his mother.
